Concierge Nursing Works
The Nurse you didn't know you need
With L&C VIPs, you have dedicated Registered Nurse who gets to know you and your family, your health history, your medications, your providers, and what matters to you. Our nurse led model provides personalized skilled nursing and ongoing support at home, with a consistent presence you and your family can trust.
Your RN provides skilled nursing care, assesses changes in your condition, helps you understand your care plan, reinforces your physician recommendations, communicates important information to your healthcare providers, and helps you navigate the healthcare system when you need it. They monitor changes in your condition, help you understand your care plan, reinforce your physician recommendations, communicate with your healthcare team, and help you navigate next steps.
When something changes, you don't have to figure it out alone or start over with someone new. You call your nurse. Your RN can assess what's happening, help identify the appropriate next step, and stay involved as things unfold.
That is the L&C VIPs difference: One dedicated RN who knows you, provides your nursing care, and leads the nursing side of your care.
The private duty nursing process at L&C VIPs is designed to remove the friction families feel when they are juggling appointments, prescriptions, and a system that rarely talks to itself. Below is exactly what to expect from your first call to ongoing care.
WHAT YOUR CONCIERGE NURSE ACTUALLY DOES
A registered nurse who takes an active role in your care not just simply checking off tasks. The role is intentionally personal and comprehensive.
Your nurse communicates with physicians and specialist, helps prepare and attend appointments when helpful. reviews your labs, manages and reviews your medications, monitors your health, and provides hands on skilled nursing care in your home.
1 The Consultation
The first step is simply a conversation.
You reach out, we listen. .We take the time to understand the person behind the diagnosis, the situation, what you or your family are navigating, and what meaningful support would look like for you.
There is no pressure and no rushed sales pitch. This is simply an opportunity to talk, ask questions, and see if concierge nursing is the right fit for your needs and if it isn't we'll tell you that, too.
2 Nursing assessment and care plan
The next step is a throrough assessment with a registered nurse. We take the time to understand the whole picture - your medical history, medications, recent hospitalizations or procedures, mobility, nutrition, daily routines. and the enviroment you live in. When helpful, we obtain medical records and communicate with your physicians so we have the information needed to truly understand your care.
From there, we develop a written, individualized care plan that reflects your needs and priorities. It outlines what your nurse will oversee, the frequency of care, which physicians and specialists to coordinate with, and goals were working toward. Your care plan is not static; it is refined as your needs, circumstances, and goals change.
3 Care, Coordination, and Continuity
Once care begins, your nurse provides the hands-on care outlined in your plan while also staying on top of the details that matter. That may include managing medications, monitoring changes in condition, communicating with your physicians, preparing for up coming appointment, and keeping your family informed.
Your care is structured around what you actually need, whether that means regular visits, focused support during recovery, or after a hospital stay. If something unexpected comes up, wheter a new diagnosis, medication change, or shift in condition your nurse helps navigate those changes and makes sure everyone is on the same page. You shouldnt have to navigate all those details on your own. We are here to take that responsibility off your plate.
4 Ongoing Reassessments
Health can shift quickly. Your nurse pays attention to those changes and respods accordingly. That may mean increasing involvement for a period of time, or recognizing when less is needed. Decisions are based on what is happening not a preset schedule.
There is no formula for how long someone should need a concierge nurse. Some clients need us for a season. Others choose to have us involved long term. There is no obligation to continue beyond what feels right to you.
